How to Refer a Participant

Initial Contact

Contact us via phone, email, or referral form to discuss the participant's needs and how we can support them.

Information Sharing

Share participant's NDIS plan and support needs (with appropriate consent) to help us understand how we can assist.

Consultation

We will arrange a consultation with the participant to discuss their needs, goals, and how our services align with their plan.

Service Agreement

If suitable, we will create a service agreement outlining the supports we will provide and how we will work together.
